TB-500 oral research capsules. TB-500 is a synthetic peptide derivative based on thymosin β4 (Tβ4) — a 43-amino-acid actin-sequestering peptide expressed at high concentration in virtually every mammalian tissue and one of the most extensively characterized regenerative peptides in modern preclinical research. The proximal active fragment, residues 17-23 (Ac-LKKTETQ-OH), retains the principal actin-binding and angiogenic activity of the parent molecule in a far smaller, more synthetically tractable peptide. Across two decades of preclinical work, TB-500 / Tβ4 has been characterized in models of cardiac, dermal, corneal, neural, hepatic and musculoskeletal injury, with progression to Phase II/III clinical trials for dermal repair (RGN-137), dry eye (RGN-259, FDA-cleared as a treatment for neurotrophic keratopathy in select markets) and wound healing.

Research Overview

Thymosin β4 (Tβ4) is the most abundant member of the β-thymosin family in mammalian cells, where its principal known function is sequestration of monomeric G-actin to maintain a polymerization-ready intracellular pool [1]. Beyond this canonical role, Tβ4 has emerged over two decades as a remarkably broad-spectrum tissue-repair signaling peptide: in well-characterized models, it accelerates epicardial-progenitor activation and infarct repair after myocardial injury; promotes dermal wound closure and reduces scarring; supports corneal epithelial healing; modulates angiogenesis through downstream VEGF signaling; and exhibits neuroprotective activity in stroke and traumatic brain injury models [1,2]. The active fragment Ac-LKKTETQ — corresponding to the actin-binding domain — replicates the principal repair activity in a chemically simpler, more synthetically tractable molecule [2]. Tβ4 has progressed through multiple Phase II clinical trials: RGN-137 for epidermolysis bullosa, RGN-259 for dry eye disease and neurotrophic keratopathy (FDA-cleared in select markets as Regenetreat), and earlier programs for chronic wound healing [3].
